- liaise with the ward, ensure necessary assessments are carried out prior to discharge, arrange necessary care.
Advocacy under Deprivation of Liberty Safeguards
- to be your representative to ensure all care and support is provided in the least restrictive way, and in your best interest.
- Challenges through the Court of Protection if required.
- Supporting you to appeal your deprivation if this is what you want to do, or approaching the Court to request care arrangements are reviewed.
Liaise with relevant organisations on your behalf.
- Care Agencies, Care Homes, Voluntary Organisations, Court of Protection, the ICB and other professionals as necessary
